Field Trip planners submit to principal for information if requested by Principal.

1. Program
 a. Relate the field program to the classroom program.
 b. Outline the purpose and objectives of the trip.
 c. Provide a brief summary of the educational activities and subjects involved in the field trip.

2. People Involved:
 a. Number of students, staff and other adults.

3. Dates and Times:
 a. Departure and return.
 b. Location(s) for departure and return.
 c. Phone chain plan — notification of delays, changes, etc.

4. Regular School Program:
 a. Coverage of classes, and other duties (yard, noon-hour, bus supervision, regularly scheduled extra-curricular activities, etc.) for teacher on the trip.
 b. Appropriate program for students who choose to stay in regular school program.
 c. Arrangements for students who miss regular classes due to participation.

5. Finances
a. Expenditures (transportation, accommodation, meals, rentals, admission fees and miscellaneous).
 b. Revenue (student levy, fundraising activities, Board support, etc.)
 c. Advise parents of fundraising ground rules.

6. Safety Procedures:
 a. Medical Facilities in the area.
 b. Medical qualifications of staff.
 c. First Aid ratios are appropriate for the number of students/participants and nature of the trip.
 d. First Aid Kit location.
 e. Staff qualifications for all potential personal risk trips.
 f. On-site safety procedures.
 g. Awareness of student health information.
 h. Appropriate medical insurance, if out of province.
 i. Supervision ratios may need to be increased when special needs students are involved.
 j. Approved vehicles are used as transport.
 k. Risks and options to mitigate have been reviewed with Principal.

7. Trip Itinerary:
 a. Accommodation — date/location/telephone numbers.
 b. Meals.
 c. Mode of transportation and route.
 d. Special arrangements/attractions/events.
 e. Plan for delays or changes in itinerary.

8. Students Personal Needs:
 a. Appropriate clothing
 b. Toiletries, etc.

9. Parents notified of risks involved

10. Parents are reminded to keep the school informed of the medical information of student(s).

11. Consent Form:
 a. Collect consent and medical forms.

12. Prior To Departure:
 a. Report final list of student participants to office.
 b. Supervisors bringing necessary student information.
