Identification Documents

In the province of Alberta, a copy of a child/student/parent/guardian identification documentation must be placed in the Student Record. The documents listed below will be accepted in the event of an enrolment audit.

If a child’s/student’s document does not prove right of access to education in Alberta, a parent document is required as well. Documents must contain the student's legal name and birthdate. 

Please ensure you provide copies of both sides of required documents.

One of the following student documents:

  • Canadian Birth Certificate with Parentage
  • Canadian Certificate of Live Birth with Parentage
  • Canadian Citizenship Card or Certificate AND Proof of Parentage
  • Canadian Certificate of Indian Status Card AND Proof of Parentage
  • Valid Canadian Passport AND Proof of Parentage
  • Valid Treaty Card AND Proof of Parentage

Student Birth Certificate with Parentage or Passport and Proof of Parentage AND the following:

  • Proof of a parent’s Canadian citizenship
  • Document linking the child to the parent

Student Birth Certificate or Passport AND one of the following options:
Note: Documents must link student to parent/legal guardian

  • Option #1
    • Valid Permanent Resident Card for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #2
    • Valid Confirmation of Permanent Residence for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #3
    • Valid Custodial Parent/Legal Guardian Permanent Resident Card AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #4
    • Valid Custodial Parent/Legal Guardian Confirmation of Permanent Residence AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian

*A Single Journey Travel Document/Single Journey Document for Resettlement to Canada may be used in place of a passport
**Legal guardianship as per the Alberta Court of King's Bench
***A student from a Visa Exempt country can provide a Electronic Travel Authorization document to prove legal entry into Canada.

Student Birth Certificate or Passport AND the following:

  • Step-parent’s proof of Permanent Residence status
  • Copy of application for permanent residency for the student and the student’s custodial parent/legal guardian, including payment receipt OR study permit for the child
  • Document linking the child to the custodial parent/legal guardian
  • Document linking proof of marriage between the custodial parent/legal guardian and the step-parent

***A student from a Visa Exempt country can provide a Electronic Travel Authorization document to prove legal entry into Canada.

Student Birth Certificate or Passport AND one of the following options:

    • Valid Canadian Visitor Record for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #2
    • Valid Student Study Permit for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #3
    • Valid Temporary Resident Visa for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #4 - If temporary residence has expired:
    • Proof of Application for Temporary Residence, including payment receipt for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #5
    • Notice of Decision for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #6
    • Valid Refugee Protection Claimant/Identity Document for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #7
    • Valid Consulate Visa for the student AND
    • Custodial parent/legal guardian’s citizen document
  • Option #8
    • Valid Custodial Parent/Legal Guardian Student Study Permit along with proof of acceptance into a minimum 2-year program from the post-secondary institution, and a receipt for payment of tuition AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #9
    • Valid Custodial Parent/Legal Guardian Work Permit AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #10
    • Valid Custodial Parent/Legal Guardian Temporary Resident Visa AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #11 - If temporary residence has expired:
    • Custodial Parent/Legal Guardian Proof of Application for Temporary Residence, including payment receipt AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #12
    • Valid Custodial Parent/Legal Guardian Refugee Protection Claimant/Identification Document AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #13
    • Custodial Parent/Legal Guardian Notice of Decision AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian
  • Option #14
    • Valid Custodial Parent/Legal Guardian Consulate Visa AND
    • Proof student is legally in Canada and connected to the custodial parent/legal guardian

To prove residency within FMPSD boundaries, we need proof of address. 
The proof of address must display a legal parent/guardian/independent student's name and current address.


Examples of accepted proof of address documents are:

  • Driver's License
  • Utility Bill
  • Bank Statement
  • GST Rebate
  • Home or Renter Insurance
  • Income Tax Statement
  • Mortgage Statement
  • Utility Bill Assessment
  • Property Tax

Note: If the student is living with a non-legal guardian within FMPSD boundaries to attend an FMPSD school, you must provide:

  • One of the above documents for the non-legal guardian
  • A completed FMPSD Consent to Guardianship document
